The Plot

The Aftermath and the Public Execution

The narrative opens with a visceral flashback immediately following the events of the first film. Dr. Lawrence Gordon, having severed his own foot to escape the bathroom trap, crawls agonizingly through a dark hallway. Desperate to survive, he discovers a steaming pipe and presses his severed stump against it, cauterizing the wound to staunch the blood loss before passing out from the overwhelming pain.

The scene shifts to a bustling public square where a storefront window reveals a gruesome spectacle to a gathering crowd. Two men, Brad and Ryan, awaken to find themselves chained to opposite ends of a worktable featuring three circular saws. Their mutual lover, Dina, who has been manipulating and cheating on both of them, is suspended directly above the center blade. Jigsaw’s puppet appears, informing them that they have 60 seconds to decide who survives. To save Dina, one man must kill the other by pushing the saws toward his opponent. If they fail to act, the central blade will rise and bisect Dina. Though the men initially battle one another, they realize Dina's deception is not worth their lives. In a unified decision, they allow the timer to expire, and Dina is lowered onto the spinning blade, slicing her in half before the horrified onlookers.

Hoffman’s Retribution and the Immunity Deal

Having survived the reverse bear trap from the previous ordeal, Mark Hoffman retreats to an abandoned hangar to tend to his wounds and plot his revenge against Jill Tuck. Jill, terrified after witnessing Hoffman’s survival, flees to the police station to seek Matt Gibson, an internal affairs officer. She reveals that Hoffman is the true successor and accomplice to her ex-husband, John Kramer, the infamous Jigsaw Killer. Desperate for protection, she offers Gibson full disclosure and evidence to incriminate Hoffman in exchange for total immunity. Gibson, who harbors a personal vendetta against Hoffman and wishes to end the Jigsaw murders, agrees to the terms.

The Horsepower Trap

Hoffman initiates a brutal new game at a scrapyard involving a group of four skinheads: Evan, Kara, Dan, and Jake. Evan, the ringleader, wakes up glued to the driver's seat of a jacked-up car. A cassette tape informs him that he has 30 seconds to tear his skin from the seat and pull a lever to stop the engine. Failure will trigger a chain reaction of death for his friends.

Evan struggles violently but fails to pull the lever in time. The car drops, crushing Kara’s head beneath the rear tire. The vehicle then accelerates forward, ripping the arms and jaw off Dan, who is chained to the rear tow bar. Jake, strapped to a garage door in the car's path, is struck and killed instantly. Finally, the car crashes through the garage door and collides with another vehicle, ejecting Evan through the windshield to his death.

The False Survivor

While the police investigate the carnage at the scrapyard—where Gibson places Jill in protective custody after finding her fingerprints on the reverse bear trap used on Hoffman—Hoffman sends a video ultimatum. He demands Jill be handed over to him, or the games will continue. Gibson refuses and secures Jill in a detention cell.

Hoffman’s primary game targets Bobby Dagen, a self-help guru who achieved fame and fortune by fabricating a story about surviving a Jigsaw trap. Bobby awakens in a suspended cage within an abandoned asylum. He is told he has 60 minutes to navigate a gauntlet of tests to save his wife, Joyce. If he fails to reach the end, she will die. The cage floor drops, leaving him hanging over a bed of spikes, but Bobby manages to swing to safety, beginning his trial.

Silence and Sacrifice

Bobby’s first test involves his publicist, Nina, who is strapped into a contraption featuring four metal spikes aimed at her throat. The mechanism is sound-sensitive; if Nina screams, the spikes move closer. The key to unlock her is attached to a fish hook inside her stomach, connected to a string Bobby must pull. He retrieves the key, but the excruciating pain forces Nina to scream repeatedly. The spikes advance with every cry, and despite Bobby’s efforts, the mechanism triggers, impaling her throat and killing her.

The Wheel of Impalement

Progressing deeper into the facility, Bobby encounters his lawyer, Suzanne, strapped to a large rotating wheel. As the wheel turns, she is slowly lowered toward three metal rods positioned to pierce her eyes and mouth. To stop the wheel, Bobby must insert himself into a device and lift a heavy bar for 30 seconds. Doing so drives metal spikes into his sides, causing immense pain. Bobby attempts to hold the weight but eventually succumbs to the agony and drops the bar. The wheel completes its rotation, and Suzanne is impaled to death before his eyes.

Blind Trust

Bobby enters a dilapidated hallway where the floor has largely collapsed, leaving only beams and stone arches. His best friend, Cale, stands on the opposite side, blinded by a mask locked onto his head. Bobby must guide Cale across the treacherous beams to reach him within 60 seconds. The key to Cale's mask hangs from the ceiling. Bobby succeeds in retrieving the key and tossing it to Cale, but in the panic and confusion, Cale fails to catch it. As the timer expires, the floor drops away completely, and Cale is hanged by the noose around his neck.

The Wisdom of Teeth

Bobby reaches a sealed metal door protected by a combination lock. The clue reveals that the combination consists of numbers etched onto two of his own wisdom teeth. In a gruesome display of determination, Bobby uses a pair of pliers to extract his own teeth, reading the numbers to unlock the door. He bursts into the final room to find his wife, Joyce, chained by the neck on a metal platform surrounded by an electric fence.

The Trojan Horse and the Slaughter

Parallel to Bobby's game, Gibson deciphers Hoffman's clues and realizes the game is taking place at the Clear Dawn Psychiatric Hospital. He dispatches a SWAT team to the asylum but heads back to the scrapyard himself after tracing Hoffman’s video signal there. The SWAT team raids the asylum, only to be trapped in a room that seals shut and fills with deadly gas, killing them all.

At the scrapyard, Gibson and two officers discover a secret surveillance room. They find the body of Dan, the skinhead from the car trap, and realize Hoffman has used the body bag to infiltrate the police station. Hoffman had hidden himself inside the corpse bag to bypass security. Before Gibson can warn the station, a sentry gun activates in the surveillance room, gunning down Gibson and his officers.

The Final Test and The Brazen Bull

Back at the asylum, Jigsaw’s final message mocks Bobby, instructing him to perform the very act he lied about surviving: driving two meat hooks through his pectoral muscles and hoisting himself up to the ceiling to deactivate the trap. Desperate to save his wife, Bobby drives the hooks into his chest and begins the ascent. He manages to connect the extension cords, but his flesh tears under his own weight, and he falls to the ground, failing the test. The timer hits zero, and a massive brazen bull capsule closes around Joyce. As Bobby watches helplessly, the device activates, roasting his wife alive.

Loose Ends

Inside the police station, Hoffman emerges from the body bag and begins a bloody rampage. He slaughters everyone in his path, including Officer Palmer and Officer Rogers, as he hunts for Jill. Jill attempts to hide but is eventually cornered and brutally beaten by Hoffman in the evidence room. He straps her into a chair and retrieves the original reverse bear trap. Unlike his own survival, Jill is given no means of escape. Hoffman activates the timer and watches as the device springs open, ripping Jill’s jaw apart and killing her instantly.

Game Over

Having exacted his revenge, Hoffman destroys his hideout and attempts to flee the city. However, as he walks away, he is ambushed by three figures in pig masks. The leader unmasks himself as Dr. Lawrence Gordon. Flashbacks reveal that after surviving the bathroom trap, Gordon was nursed back to health by John Kramer and became a secret accomplice, aiding in the preparation of traps for years. complying with John’s final request to protect Jill, Gordon abducts Hoffman.

Hoffman awakens in the iconic underground bathroom from the first film. Dr. Gordon shackles Hoffman’s ankle to the pipes, just as he was once chained. He then picks up the hacksaw he used to sever his own foot, but instead of leaving it for Hoffman, he throws it out into the hallway. Gordon turns off the lights and seals the door, leaving Hoffman to die in the darkness with the final declaration: "Game Over."
