The Plot

The Survival of Dr. Gordon

The narrative opens with a visceral flashback sequence occurring immediately after the events of the first film. Dr. Lawrence Gordon, having just severed his own foot to escape the industrial bathroom, crawls agonizingly through the dimly lit hallway. Desperate to stem the profuse bleeding from his severed limb, he drags himself toward a steaming pipe against the wall. With a scream of agony, Gordon presses his stump against the scalding metal, cauterizing the wound in a cloud of burning flesh and steam before succumbing to the pain and losing consciousness in the darkness.

The Public Execution

The scene shifts to a modern timeline, centering on a shop window in a busy public square where a horrific display attracts a growing crowd of horrified onlookers. Two young men, Brad and Ryan, awaken to find themselves chained to a workbench equipped with three buzz saws. Suspended above them is Dina, a woman they were both dating simultaneously, unaware of her infidelity. Jigsaw’s puppet appears, revealing the rules: Dina has manipulated both men, and now one of them must die within 60 seconds.

The mechanism is designed such that Brad and Ryan must push the saw unit toward their opponent to save Dina from being lowered onto a third, central blade. Initially, the men fight violently against one another, fueled by adrenaline and the desire to save the woman they love. However, as Dina pleads for her life, her duplicity becomes clear. Realizing she is not worth their sacrifice, Brad and Ryan form a silent pact. They cease fighting each other and allow the central mechanism to center itself. As the timer hits zero, Dina is lowered onto the spinning blade and bisected before the eyes of the screaming crowd outside.

The New Jigsaw and the Internal Affairs Deal

Following his survival of the reverse bear trap at the conclusion of the previous massacre, Mark Hoffman retreats to an abandoned hangar to tend to his facial wounds and plot his vengeance. Meanwhile, Jill Tuck, having failed to eliminate Hoffman, rushes to the police station in a panic. She demands to speak with Matt Gibson, an Internal Affairs officer with a complicated history involving Hoffman. Jill reveals the explosive truth: Mark Hoffman is the successor and accomplice to her ex-husband, John Kramer.

In exchange for full immunity and immediate protection, Jill offers Gibson comprehensive evidence to convict Hoffman and end the Jigsaw legacy. Eager to close the case that has plagued the city, Gibson accepts the deal and places Jill into protective custody, unaware that Hoffman is already initiating his next phase of terror.

The Horsepower Trap

Hoffman targets a group of four skinheads—Evan, Kara, Dan, and Jake—abducting them to a derelict junkyard for a brutal multi-layered test. Evan, the ringleader, wakes up glued to the driver's seat of a jacked-up car. A cassette tape informs him that he has 30 seconds to tear his skin from the seat and pull a lever to stop the engine. Failure will result in a chain reaction of death for his friends.

Evan struggles frantically, ripping the skin from his back, but fails to reach the lever in time. The car accelerates, knocking the jacks away. The spinning rear tires descend, crushing the head of Kara, who is strapped beneath the wheel. As the car shoots forward, it rips the arms and lower jaw off of Dan, who is chained to the rear bumper. The vehicle then smashes into Jake, who is tied to a garage door, obliterating him. Finally, the car crashes through the door and collides with another vehicle, ejecting Evan through the windshield to his death.

The Ultimatum and the False Hero

Gibson and his partner, Officer Rogers, arrive at the junkyard carnage. They discover the reverse bear trap Jill used on Hoffman, now covered in her fingerprints, which Gibson uses to justify keeping her in a holding cell for her safety. Hoffman, watching from the shadows, sends a video ultimatum to Gibson: hand over Jill Tuck, or the games will continue. Gibson refuses, choosing instead to move Jill to a high-security detention cell.

Simultaneously, a new game begins for Bobby Dagen, a self-help guru who rose to fame and fortune by falsifying a story about surviving a Jigsaw trap. Bobby awakens in a suspended cylindrical cage above a floor of spikes. He is told that he has 60 minutes to traverse the asylum and complete a series of tests to save his wife, Joyce. If he fails, she will die. Bobby manages to escape the cage just before the floor drops, beginning his journey through the derelict facility.

Tests of Silence and Sight

In his first trial, Bobby finds his publicist, Nina, in the "Silence Circle." She is strapped to a chair surrounded by four spears aimed at her throat. The key to her release is attached to a fishing line hooked inside her stomach. Bobby must pull the hook out through her throat, but the device is sound-sensitive; if Nina screams, the spears will advance. Despite Bobby’s efforts to extract the key, Nina’s screams of agony accelerate the timer, and the spears impale her throat, killing her instantly.

Proceeding to the next sector, Bobby encounters his lawyer, Suzanne, strapped to a large rotating wheel. To save her from being impaled by rods positioned to pierce her eyes and mouth, Bobby must physically lift a heavy metal gate, which in turn drives spikes into his own ribs. He endures the torture for several seconds, but the pain becomes unbearable. He collapses, dropping the gate, and watches helplessly as the wheel completes its rotation, killing Suzanne.

The Blind Leading the Blind

Bobby enters a dilapidated hallway where the floor has largely collapsed, leaving only narrow beams. At the far end stands his best friend, Cale, who is wearing a heavy mask that blinds him. A key hangs from the ceiling in the center of the chasm. Bobby must verbally guide Cale across the treacherous beams to the key within 60 seconds.

The tension mounts as Cale stumbles blindly over the abyss. Bobby manages to retrieve the key and throws it to Cale, but in the panic, Cale fails to catch it. The key falls into the darkness below. As the timer expires, the winch system activates, dragging Cale upward by the neck and hanging him.

The Wisdom Teeth and the Final Reveal

Nearing the end of his game, Bobby encounters a locked metal door. The combination is etched onto two of his own wisdom teeth. In a gruesome act of self-mutilation, Bobby uses pliers to rip the teeth from his jaw to read the numbers. He unlocks the door and enters the final chamber, where he finds his wife, Joyce, chained by the neck on a metal platform surrounded by electric fences.

A final video from Jigsaw exposes Bobby’s lies to Joyce. To save her, he must now perform the very feat he lied about surviving: he must drive two meat hooks through his pectoral muscles, hoist himself to the ceiling, and connect an extension cord to deactivate the trap.

The Decoy and the Body Bag

While Bobby plays his game, Gibson deciphers a clue in Hoffman’s video message leading him to the Crossroads Manufactory, the site of the asylum. Suspecting this is Hoffman's command center, Gibson raids the building with a SWAT team. However, Officer Palmer informs him via radio that the video signal is actually tracing back to the junkyard. Realizing he has been lured away, Gibson leaves the SWAT team behind—who are subsequently trapped and killed in a gas chamber—and races back to the junkyard.

At the junkyard, Gibson and two officers find a hidden surveillance room. They discover the body of Dan, the skinhead, is missing from its body bag. Watching the monitors, Gibson realizes with horror that Hoffman swapped places with the corpse to infiltrate the police station from within the morgue. Before Gibson can warn the station, a sentry gun activates in the room, gunning down Gibson and his men.

The Massacre and the Oven

Back at the asylum, Bobby drives the hooks into his chest and attempts to hoist himself up. He agonizingly connects the cords, but his pectoral muscles, unable to support his body weight, rip apart. He falls to the floor, defeated. As the timer hits zero, a massive brazen bull oven encloses Joyce. She is incinerated alive while Bobby watches, screaming in futility.

Inside the police station, Hoffman rises from the body bag in the morgue. He initiates a brutal rampage, killing Officer Palmer, Officer Rogers, and anyone else in his path. He eventually corners Jill Tuck in the evidence room. Despite her desperate struggle, Hoffman knocks her unconscious and straps her into the original Reverse Bear Trap. With no key and no time to escape, the device activates, ripping Jill’s jaw apart and killing her instantly.

Game Over

Hoffman destroys his hangar and prepares to flee the city, believing his work is done. However, as he walks towards a getaway car, he is ambushed by three figures in pig masks. The leader removes his mask to reveal Dr. Lawrence Gordon, alive and walking with a prosthetic foot.

A montage of flashbacks reveals that Jigsaw found Gordon after the bathroom game, nursed him back to health, and recruited him as a secret asset. Gordon had been working in the shadows for years, performing the complex medical procedures required for the traps. Following John Kramer's final request to "watch over Jill," Gordon acts to punish Hoffman. He drags Hoffman into the iconic bathroom from the first film, chains him to the pipes, and throws the hacksaw out of reach. As Hoffman screams, Gordon turns off the lights and seals the door, declaring, "Game Over."
