The Plot
Escape from the Bathroom
Picking up immediately following the events of the previous film, Detective Eric Matthews finds himself trapped in the dilapidated subterranean bathroom, chained by the ankle to a pipe by Amanda Young. Panic and desperation set in as he realizes the dire nature of his situation. Refusing to succumb to a slow death, Eric takes drastic measures to free himself. He grabs a heavy toilet tank lid and brutally smashes his own foot, shattering the bone. Through sheer agony and determination, he manages to slip his broken foot through the shackle, escaping his confinement to search for a way out of the tunnels.
The Impossible Trap
Six months later, the grisly legacy of the Jigsaw Killer continues. Lieutenant Daniel Rigg, along with Detectives Allison Kerry and Mark Hoffman, are called to a crime scene at an elementary school. They discover a victim named Troy, who has been brutally killed in a classroom trap. Troy had been tasked with ripping chains out of his own flesh—pierced through his hands, shoulders, and jaw—to escape before a nail bomb detonated. However, upon closer inspection, Detective Kerry notices a disturbing anomaly: the room's only exit had been welded shut. This crucial detail leads her to the chilling conclusion that this trap was never meant to be survivable, suggesting that someone other than John Kramer is now orchestrating the games, violating Jigsaw's core philosophy of providing a chance for survival.
The Angel Trap
Soon after the investigation, Kerry is abducted from her apartment and awakens suspended in a harness inside an industrial device known as the "Angel Trap." A video tape informs her that she has one minute to retrieve a key from a beaker of acid suspended before her to unlock the mechanism hooked into her rib cage. Despite the searing pain, Kerry plunges her hand into the acid and successfully retrieves the key. She unlocks the device just as the timer hits zero, but the trap does not release. Realizing with horror that her game has also been rigged, she can only scream as the machine springs open, tearing her rib cage apart and killing her instantly.
The Surgeon and the Dying Master
Elsewhere, Amanda Young abducts Dr. Lynn Denlon, a depressed trauma surgeon whose marriage is failing. Lynn is taken to an abandoned meatpacking plant which serves as the new hideout for Jigsaw operations. She is brought before John Kramer, who is now bedridden and in the final stages of his terminal cancer. John presents Lynn with an ultimatum: she must keep him alive long enough for another test subject to complete their game. To ensure her compliance, Amanda locks a complex collar around Lynn’s neck. This device is wirelessly connected to John’s heart monitor; if John’s heart stops beating, or if Lynn attempts to move out of range, the collar will detonate, blowing her head off.
Jeff’s Game Begins
Simultaneously, a man named Jeff wakes up inside a wooden crate within another section of the plant. Jeff is a grieving father who has been consumed by vengeance since the death of his son, Dylan, in a car accident three years prior. He has neglected his wife and daughter, Corbett, in his obsession with the people connected to the accident. A tape left by Jigsaw informs Jeff that he will face a series of tests that will lead him to "the man responsible for the loss of his child," offering him a choice between forgiveness and revenge.
The Freezer Room
Jeff enters the first chamber, a freezing room where he finds Danica Scott, the only witness to his son's accident, chained naked within a freezer frame. Danica had refused to testify against the driver, leading to a light sentence. As the test begins, she is sprayed with water which instantly begins to freeze on her skin due to the sub-zero temperatures. Jeff can save her by reaching for a key behind frozen metal bars, but he hesitates, tormented by her inaction in the past. Eventually, Danica manages to convince him to help, but his hesitation proves fatal. By the time Jeff retrieves the key, Danica has frozen to death, encased in ice.
The Pig Vat
Moving deeper into the plant, Jeff encounters his second test involving Judge Halden, the man who presided over the court case and gave the driver a lenient sentence. Halden is chained to the bottom of a deep vat, which slowly fills with a slurry of liquefied, rotting pig intestines. To retrieve the key and stop the flow, Jeff must incinerate his deceased son's toys and possessions inside a furnace. The act requires Jeff to physically let go of his painful memories. After a struggle with his emotions, Jeff activates the incinerator, destroying the mementos, and retrieves the key in time to pull the Judge from the vile filth, saving his life.
The Rack
Jeff and the rescued Judge Halden proceed to the third test, where they find Timothy Young, the drunk driver who killed Dylan. Timothy is strapped into "The Rack," a mechanical device designed to slowly twist his limbs and head until they snap. The key to the device is tied to the trigger of an enclosed shotgun. Despite his overwhelming hatred, Jeff attempts to save Timothy. During the chaotic attempt to retrieve the key, the shotgun discharges, and Judge Halden, who had stepped into the line of fire, is shot in the head and killed. Jeff frantically tries to unlock the device as Timothy’s limbs are broken one by one. Ultimately, Jeff is too late; the machine twists Timothy's head around, breaking his neck and killing him.
Surgery and Betrayal
Back in the makeshift sickroom, John’s condition deteriorates rapidly, forcing Dr. Lynn Denlon to perform an emergency decompressive craniectomy to relieve pressure on his brain. During the crude surgery, a semi-conscious John hallucinates and professes his love for a woman, whom Amanda mistakes for Lynn. Consumed by jealousy and emotional instability, Amanda retreats to cut herself. Flashbacks reveal her history as John's apprentice, showing that she mercy-killed Adam Stanheight in the bathroom after the first film and had been manipulating traps because she did not believe the victims could truly change.
The Final Confrontation
Following the surgery, Amanda finds a letter in her desk drawer that leaves her visibly distraught. She returns to the sickroom just as Jeff completes his third test. John instructs Amanda to remove Lynn's collar, declaring that Lynn has fulfilled her duty. However, Amanda refuses. She draws a gun on Lynn, ranting about her disillusionment with Jigsaw's philosophy. She confesses to rigging Troy's and Kerry’s traps to be inescapable and reveals that she fought Eric Matthews after he escaped the bathroom, leaving him for dead. John pleads with her, warning that her own life depends on Lynn's survival, but Amanda is too far gone in her rage.
Collateral Damage
Just as Jeff enters the room, Amanda shoots Lynn in the back. In immediate retaliation, Jeff shoots Amanda in the neck with a gun he found during his tests. As Amanda bleeds out, a shocking truth is revealed: Jeff and Lynn are husband and wife. John then reveals that the true test was for Amanda all along; he knew she was sabotaging the games and wanted to see if she was capable of granting life to someone she hated. By shooting Lynn, she failed. Amanda dies from her wound as the couple is briefly reunited.
Forgiveness or Vengeance
With Amanda dead, Jeff turns his weapon on John, attempting to kill him, but the gun is empty. John remains calm and offers Jeff one final test: he can exact his revenge and kill John, or he can forgive him. John promises that if Jeff forgives him, he will call an ambulance to save Lynn, who is rapidly losing blood. Jeff stands over the dying Jigsaw and says, "I forgive you." However, in a twist of irony, Jeff immediately picks up a power saw and slashes John’s throat.
Game Over
As John's throat is slit, the door to the room automatically slams shut and locks. A dying John pulls out a tape recorder and plays a final message. The tape informs Jeff that by killing John, he has failed his final test. It reveals that Jeff’s daughter, Corbett, has been abducted and locked in a room with a limited air supply, and John was the only person who knew her location. Jeff must now play a new game to find her. As the recording ends, the heart monitor flatlines as John dies. The signal triggers Lynn’s collar, which detonates instantly, killing her. Jeff is left screaming in the sealed room, surrounded by corpses, having lost everything.
